A vacancy for a Care Coordinator has arisen in our busy practice to support our patients and be directly involved with the Anticipatory Care requirements as well as attending the relevant Anticipatory Care Multidisciplinary Team meetings as well as assisting with other non-clinical needs of the patient.

There is a requirement to visit care homes as well as patients in order to assess patient needs so that appropriate referrals can be made to provide additional support to the patients.

This is a permanent role based on 37.5 hours per week with NHS pension.

The clinical team comprises of 5 GP Partners, 6 Salaried GP’s and 2 Paramedic Practitioner, and the management team comprises of a Practice Manager, Assistant Practice Manager and a Reception Supervisor.

You need to be able to work effectively within a busy team as well as independently and have good communication skills, and ideally have experience with EMIS Web, although training can be provided.

Job Summary

A patient care co-ordinator (PCC) may be required to deal with patients and, if appropriate, their carer, before or after the patient’s consultation with a clinician or other healthcare professional.

Working closely with the patient and their clinician or other healthcare professional, the PCC co-ordinates patients’ healthcare and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

The PCC’s role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician associates, paramedics, health and wellbeing coaches, podiatrists, occupational therapists and pharmacy technicians.

To identify patients that require responsive teams. Focusing on managing disease and preventing further ill health. Helping to transform care from crisis and disjointed to proactive and coordinated care.

Primary Responsibilities

The following are the core responsibilities of the Care Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels and the availability of a social prescribing link worker within the PCN:

a. To work closely with practice and PCN healthcare roles, the PCC is to identify and work with a cohort of people to support their personalised care requirements

b. To collate all of a patient’s identified care and support needs and review the options to meet these needs

c. To help people to manage their needs by answering their queries and supporting them in making appointments

d. To support people to access appropriate benefits

e. To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools

f. To provide patients with high quality, easy to understand information to assist them in making choices about their care

g. To support patients in understanding their level of knowledge, skills and confidence (known as “activation” level) when participating in their health and well-being.

h. To liaise with other PCCs in other practices within the PCN and share best practice

i. Where appropriate, to assist patients to access personal health budgets

j. To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services, where appropriate working hand in hand with social prescribing link workers (SPLW)

k. To support in the delivery of enhanced services and other service requirements on behalf of the PCN

l. To attend and participate in the delivery of multi-disciplinary teams (MDT) within PCNs.

m. To undertake all mandatory training and induction programmes

n. To contribute to and embrace the spectrum of clinical governance

o. To develop yourself and the role through participation in training and service redesign activities

p. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

q. To contribute to public health campaigns (e.g. flu clinics) through advice or direct care

Secondary Responsibilities

In addition to the primary responsibilities, the PCC may be requested to:

a. Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives

b. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner

c. Duties may vary from time to time without changing the general character of the post or the level of responsibility

Person Specification

· Educated to GCSE level or equivalent

· Passed training requirements as outlined by the Personalised Care Institute (desirable)

· Experience working in a healthcare or Social care setting

· Experience working in General Practice (desirable}

· Own transport

· Excellent communication skills

· Strong IT skills

· Excellent time management

· Ability to work as a team member as well as autonomously

· Be entitled to live and work within the UK as we are unable to sponsor

· Satisfactory DBS Check

· Satisfactory references (at least 2)

Please note that we are not located close to Guildford Station and although there is a bus service we recommend that candidates have a car.

The role will be weekdays – Monday to Friday.

NHS pension and a competitive salary.
