An exciting opportunity has arisen for a Full-time (37.5 hours) General Practice
Nurse to join Wideopen Medical Centre located in North Tyneside. The practice operates over two sites and has a practice population of 7780 patients.

We are looking for a motivated and innovative Practice Nurse with experience in General Practice to become part of a supportive multi-disciplinary team, using your skills, experiences and to help define and deliver safe and effective nursing care.

Main duties of the job

General Practice Nursing is varied and rewarding. You will be involved in delivering care to our population across several clinical areas. This will include cervical cytology, immunisations, and chronic disease management, including Diabetes,
Asthma and COPD. We place a high value on team working and you will be supported by your nurse colleagues and GPs as well as the wider practice team including pharmacists and physician associate.

Job responsibilities

To contribute to the successful delivery of high-quality nursing services to a defined population. This will include assessment, health promotion, chronic disease management, health screening and treatment in line with the professional competence of the post holder. To be involved with the practice delivery of National
Service Framework targets and local Health Improvement priorities. Responsibilities to maintaining and improving QoF and clinical components set by NHSE / PCNs.

Clinical

Provide general nursing care to practice population who are referred or self-refer in the practice including assessment, planning, delivery and evaluation of:

  • Wound management / post-operative wound care
  • Ear care
  • Cervical cytology
  • Help the practice develop and deliver specialist primary nurse led services including chronic disease management. Initiate the role of diagnosis for patients suspected to have a chronic disease e.g., diabetes, COPD, asthma and CHD, referring to other clinicians as appropriate.
  • Manage programs of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff.
  • Provide comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, sun protection, food hygiene, first aid and emergency medication, health insurance and vaccinations.
  • Provide a caring, confidential and considerate sexual health service including contraception and STD screening.
  • Administer child and adult immunisations and vaccinations in accordance with national and local programs. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained.
  • Undertake autonomous comprehensive assessment of patient need and develop care plan in accordance with local and national protocols e.g., blood pressure, venepuncture and other physiological measurements.
  • Make judgements on a range of complex issues requiring analysis, assessment and solution.
  • Deliver care to specialist groups in accordance with practice protocols and

QOF guidance including, local and national guidance.

  • Autonomously refer to multidisciplinary team for referral/ treatment/ discharge.
  • Safely use and manipulate equipment used in examination and treatments.
  • Computer literate with practice patient record system and electronic communication systems.
  • Safely manage hazardous biological substances including blood / urine / vomit / faeces / vaginal secretions.
  • Communicate effectively with patients, relatives and carers regarding treatment and progress of patients, maintaining confidentiality of all patient information.
  • Manage issues arising from sensitive situations e.g., unwanted pregnancy, sexually transmitted infection, child protection issues and mental health.
  • Breaking bad news and communicating with patients with life threatening and distressing diagnoses and the bereaved.
  • Act as patient advocate/chaperone.
  • Awareness and provision of evidence-based care.
  • Physically handle patients to assess / treat in emergency situations.
  • Facilitate and oversee the regulatory requirements of clinical audits and training within the HCA and Nursing Team e.g., CQC policies and procedures.

Teaching and Mentoring Role

  • The successful candidate should be able to demonstrate a learning environment for patients, nurses and other professionals.
  • Undertake regular appraisals for the practice nursing team and act as a mentor for more junior staff and students, assessing competence against stet standards.
  • Disseminate learning and information to other team members in order to share good practice and inform others about current and future devolvement's.

  • Registered Nurse
  • Qualified Cervical smear sample taker

Desirable

  • SystmOne knowledge
  • Immunisation training

Experience

Essential

  • Must have experience of working in General Practice
  • Experience of chronic disease management
  • Experience of managing a nursing team
