Lincoln Renal Community Team
An opportunity has arisen within the Lincoln Renal Community Team, for a Band 5 Nurse. You will need to be a 1stlevel registered nurse with at least 18 months’ post registration experience and preferably at least 1 year in renal nursing.
The Renal Community Team is an innovative and forward thinking area, and offers a unique opportunity to further develop specialist nursing skills.
The successful candidate will work as part of the Lincoln renal community team of nurses planning and delivering high quality care for a wide variety of renal patients, this will include delivering pre-dialysis education, education and training patients on peritoneal dialysis and home haemodialysis, supporting patients and their family in the community and assist with day case activities.
You will be fully supported by the Team Leader and all members of experienced nurses within Lincoln renal community team.
Evidence of continuous professional development within the last 12 months is essential along with recent experience and proficiency in 1 or more forms of renal replacement therapy.
A flexible working policy is offered to meet the needs of both the service and staff . The core hours of the team are 8.30am to 18.30pm Monday to Friday. Flexibility with in these times is a possibility. You will need to have access to a vehicle during working hours. Fuel cost will be reimbursed. A lease car maybe available on request.
Responsible for assessing, planning, implementing and evaluating programmes of evidenced-based nursing care to a group of patients.
Provide advice, support and information to patients and their family or carers as required and within the limits of the post holders knowledge and experience. To Liaise closely with senior members of the community team.
Assisting and contributing in the management and organisation of a community team in the clinical area and community setting area.
Actively contribute to setting and maintaining high standards of quality nursing care.
Actively participate in the education, development and supervision of other staff members.
Act as a clinical and professional role model, assessing and supporting students and learners on placement within the clinical area.
Work collaboratively and co-operatively with others to meet the needs of patients and their families, ensuring the health education and health promotion needs of patients are identifies.
Organise and perform investigations for patients in the community setting, support staff members is providing a ‘drop-in’ renal day case service and training undertaken on the unit in the training room in accordance with directorate policy and protocols.
