Our integrated teams of Nursing and Allied Health Professionals deliver care to people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in the patient’s own home, including care homes, enabling them to be independent, self-caring and live their optimum lives, reducing hospital admissions.

Services range fromcommunity nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry.

Supporting the efficient running of these services are administrative and managerial roles. All roles, at all levels, are vital in transforming community health services, ensuring high quality care is provided both now and into the future based around the needs of our patients - together we all make a difference.

Have you considered a career as a Specialist Public Health Practitioner?

Although this is a changing time for the profession. Specialist Practitioners are at the forefront by contributing to improving health outcomes for children & young people aged 0-19.

Community Services are offering the opportunity for suitable candidates to access the Specialist Community Public Health Nurse programme leading to a qualification in School Nursing. This offer is for the year of September 2023 to September 20234 on a fixed term contract.

The course is 50% in practice and 50% in University. We work in partnership with Derby University.

The week is generally split between practice days spent with an experienced Public Health Practitioner working within their defined caseload and learning about 0-19 HFT School Nursing practice and attending University. You will be assessed throughout the course both in practice and academically.

Are you: -

A registered nurse with a range of post basic qualification experience? –

Have a Diploma in Higher Education (Nursing) or equivalent? (Minimum of 120 credits at 5)

• Able to evidence professional development in your clinical area?
• Able to work effectively as a team member and independently, both within the practice setting and at University?
• Able to communicate effectively with children, young people and their families plus all statutory, voluntary and independent organisations.
• Ready to develop your clinical leadership skills to improve healthcare delivery? With a good knowledge of current national government policies and the ability to demonstrate their application to practice.
• Able to demonstrate your understanding of current practice and policy issues relating to your chosen area of practice?
• To work in collaboration with children, young people and their families, Clinical Commissioning Groups, and all statutory, voluntary and independent organisations?
• Able to clearly demonstrate the ability to manage time and resources effectively in both university and practice settings.
