Northumbria Virtual Ward service Clinical Lead
This is a fantastic and exciting opportunity to work in one of the most beautiful counties in England and be at the cutting edge of delivering acute
care to patients in their own home

The Community Response Team of staff will deliver the virtual ward service to provide care to patients in their own home who would otherwise have to have been admitted to hospital or have a longer stay.

The Virtual Ward service will offer initially pathways for community , respiratory, heart failure, post operative pathways and frailty patients. These areas will expand as the service becomes more established.

This post will be subject to assessment centre and/or psychometric testing.
Main duties of the job

To be the day to day clinical lead for the virtual wards with a focus on the community wards, to be a decision maker for admissions to and capacity of the virtual wards.

To assist with the strategic development of the Virtual Wards

To build relationships with the acute teams and provide a comprehensive clinical service for Virtual Ward patients as part of the virtual
ward team with an emphasis on patient centred care.

To lead on the care of patient in the virtual ward and to support and community nursing team with their care.

To be responsible and accountable for the physical delivery of nursing care to complex patients, in the patient's usual place of residence.

  • Ensure the highest standard of safe clinical care through the provision of effective leadership and management of clinical staff.
  • Responsible to the Senior Clinical Lead and OSM. The post holder will work within the Community Response Team delivering the Virtual Ward Service
  • Responsible for coordinating day to day care, and ensuring quality and effectiveness of nursing services within - Virtual Ward
  • Line manages Specialist Nurses, Community Nurses and Carers within the Virtual ward team
  • Provide visible, accessible and authoritative presence in the Virtual ward Service ensuring clients and their families and members of the multidisciplinary team can gain assistance, advice and support.
  • Works within the Triage rota receiving referrals from colleagues and other organisations. To use this information to decide the appropriate level of response from the community response team
  • Triage referrals for the Virtual ward, Community response service and communicate with referrers regarding the suitability of the patient for virtual ward admission.
  • Respond to enquiries from relatives/carers and other organisations and services.
  • Deal with concerns and actively problem solves to improve the patient experience, investigate and respond to incidents and complaints within target timescales.
  • Take a lead role in developing and maintaining relationships with primary and secondary care
Take shared responsibility with the Senior Management team for Clinical Governance

choice and control over their care to help them to live independently at home and avoid hospital admission where

  • Provide professional and clinical leadership and supervision to allocated teams including provision of supportive advice and to understand the activity associated with patient care.
  • Take responsibility for various components of the Clinical Governance agenda including achieving standards relevant to the areas.
  • Liaise with clinical colleagues in the continued development and re-organisation of services emphasising a patient focused approach utilising pathways of care and for delivering evidence based practice.
  • Take responsibility for ‘pure flexi staff’ (bankstaff) working in your area where appropriate.
  • Professionally responsible for the delivery of a high quality safe service for patients and carers.
  • Advise on and actively contribute to the achievement of national and local performance targets within area.
  • Responsible for maintaining all nursing and clinical care standards in line with relevant professional, Trust and NICE guidelines.
  • Work with partner organisations and advise on service developments that affect patient care.
  • Provide education and training that is accessed by a broad range of health professionals and by employees.
  • Act as a role model for all community nursing teams and to encourage the recruitment and retention of staff.
  • Facilitate the development of clinical practice by providing clinical and nursing leadership.
