JOB BRIEF

We are looking for high calibre social care assistants (or a newly qualified social worker) with a minimum of two years experience of working with adults in a local authority setting to join our fast-paced front door assessment team. Applicants who have worked in a duty service, a Reablement team or a hospital social work team will have the requisite skills. The community-based team that we are recruiting to supports residents to carry on living as independently as possible in their own homes. This role is suitable for an unqualified social care assessor or we can consider newly qualified SWs who are looking for a placement that will help them to develop their skills, provided that they have carried out Care Act assessments before.

Responsibilities

• · Engage with individuals, carers, families, groups and communities to assess and intervene preventatively using your knowledge and skills, and develop person-led Support Plans to meet the needs of individuals that promote choice, control and independence.
• · Be able to use a strength-based approach to assessment to support individuals to identify and develop their support networks and consider positive risk-taking to enhance their wellbeing.
• · Be able to identify safeguarding concerns if vulnerable residents have experienced harm, and discuss with your line manager if a qualified social worker needs to undertake section 42 enquiries in cases where the safeguarding threshold is met.
• · Be responsible for delivering high quality and legally compliant Adult Social Care support in a timely way to agreed standards.
• Be able to complete “court fit” mental capacity assessments and engage residents and their families in best interest decision-making where required

Salary: £14.91-£29.82 per hour

Ability to commute/relocate:

• Walthamstow, E17: reliably commute or plan to relocate before starting work (required)
