CLCH is one of the UK's largest NHS Trusts and an opportunity has become available to work for the Harrow Short Term Rehab, Inpatient rehabilitation, and Discharge to Assess Team as a Band 3 Rehab Support Worker. We are currently looking to recruit an experienced Support Worker that is passionate about providing high levels of care and support to individuals with provide rehabilitation support to patients.

The post will have the opportunity to work in an Integrated care setting working alongside physiotherapists and occupational therapists and supporting the team in the delivery of care to the Therapy caseload. The post holder will work individually with patients supporting them in achieving therapy goals through improving functional ability, carrying out exercise programmes with patients and working collaboratively with the physiotherapists and occupational therapists in helping patient goals be achieved.

The post holder will be given support and training to broaden skills and progress knowledge. As well as a chance to work with different people in different areas within the service.

As a Rehabilitation Support Worker you will be involved in meeting the therapy needs of a diverse group of service users with varying degrees of mental and physical issues. You will be caring and attentive with the skills to build strong and trusting relationships.

You will also work well in a team as you will be required to work alongside qualified nurses and staff from other professions to support with the delivery of excellent care to some of the most vulnerable patients.

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.
  • To carry out pre-defined rehabilitation programmes with patients in their own homes, day centres, rehabilitation units, the local environment, including outdoors and on public transport, and within the therapy centre as directed by a qualified therapist for a range of patients, including those with complex presentations.
  • To report to an allocated supervisor about treatments undertaken, progress made, outcomes of intervention and to discuss when modification of rehabilitation programmes may be necessary as part of a structured clinical supervision system.
  • To ensure all aspects of your work are of a high clinical standard and be accountable for this to the team lead, through the qualified staff.
  • To observe any progress, deterioration or other significant changes in a patient’s condition or performance and to report these to the relevant therapist involved in the client’s care.
  • To participate in the delivery of patient education programmes.
  • To undertake the measurement and evaluation of your intervention through the use of treatment plans, goals, outcome measures and clinical supervision with a qualified therapist in order to progress treatment programmes and ensure clinical effectiveness.
  • To work in partnership with patients, carers, and the multidisciplinary team to ensure a timely, coordinated, and individualised approach to intervention which respects patients’ choices and values.
  • To empower patients and their carers to take responsibility for self-management during treatment period and following discharge, using knowledge of local resources to facilitate this as appropriate
  • To organise clinical caseload in consultation with colleagues taking into account management of waiting lists and necessary nonclinical duties
  • To assemble and adjust and use specialist equipment within your competencies e.g., walking aids, standing frames, hoists, orthoses, and footwear etc and provide advice to patients / carers to ensure safe use
  • Support patients by delivering health promotion within your competencies
  • To support personal care as required to promote independence/rehabilitation in patients.
