We are looking for exceptional Band 3 Health Care Assistant to join us in our Central London Community Nursing Day Service, the service is run from 07.00am to 17.30pm.The team cover The borough of Wandsworth, crossing North, East, South and West Localities.You should hold NVQ level 3 in health and social care and have a minimum of 12 months acute or community experience.If you are a motivated team player and want to excel in delivering exceptional holistic healthcare to a diverse patient group in their own home, we want to hear from you.

Previous experience of working in the NHS is essential, community experience is desirable but training will be given.
As a Health Care assistant, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.

Responsible for the delivery of care and for managing service provision in accordance with national and local priorities, organisations policies, procedures and guidelines and in accordance with the Nursing and Midwifery Council.
  • To assist with providing nursing care as delegated by the District Nurse within patients homes
  • To provide clerical and administrative support to the team as delegated by the District Nurse/ Team Leader.
  • To deliver care in accordance with Organisation Policies, procedures and guidelines, according to competencies, as delegated by the District Nurse.

  • To maintain Knowledge, skills and attitudes necessary to deliver a quality service that are responsive to the changing needs of patients.
  • To report any changes in condition of the patient to the District Nurse/ Team Leader or in their absence the designated nurse in charge of the caseload.
  • To document accurate and concise information on all care given to patient in the community health records.
  • To be aware of Data Protection Act, and follow local codes of practice to ensure appropriate action is taken to safeguard confidential information.
  • To assist with continence re-assessments, following agreed training in competencies, reporting to the nurse in charge any changes in care packages required -Please see attached Job Description and Person Specification for full roles and responsibilities
